Existing Patients
If you already have a BIM patient portal account, click the Patient Portal button and log in using the email address associated with your account.
If you are having trouble accessing your account, please contact the BIM care team for assistance.
New Patients
If you are a new patient and have been instructed to access the patient portal, please use the email address you provided during enrollment or registration.
Follow any instructions sent to you by the BIM care team to complete your account setup.
Complete Requested Forms
Your care team may direct you to the patient portal to complete required intake forms, consent forms, health questionnaires, treatment updates, or other requested documentation.
Please complete all requested items before your scheduled appointment whenever possible.
